Early Help for Adults Navigator

Back on Track is a leading specialist provider of community learning and holistic support to adults experiencing multiple disadvantage including adults who have experience of problematic drug/alcohol use, the criminal justice system, challenges with their mental health, or those living in unstable accommodation. 

As part of the commissioned services that we deliver, we are looking for a motivated, dynamic, Early Helps for Adults navigator to deliver excellent 1:1 support to adults throughout Manchester. 

This work is carried out in partnership with Shelter and The Big Life Group and is underpinned by the key principles of early intervention and prevention. 

Your support will be person centred and trauma informed and will support participants to address challenges and reduce barriers to living well. Partnership working is an important part of this role. 

We are looking for someone who is proactive and flexible with good time management and organisational skills. You will demonstrate a commitment to the people on the programme, putting their needs at the centre of your work.

This role will be based in the Back on Track centre in Ancoats (M4 5JW). 

Key Responsibilities

Build trusted relationships with participants and support them to build more fulfilled lives:

· Provide person-centred, trauma-informed one-to-one support to people on the programme

· Identify and develop referral pathways for the benefit of those on the programme and the programme more widely.

· Make appropriate referrals into specialist support services such as housing, benefits, debt advice etc.

· Strive to find approaches to working with people who may face barriers to their engagement including offering outreach support and home visits.

· Act as an advocate, empowering people to make decisions and become more independent

· Feed into the Early Help for Adults and Back on Track safety planning and safeguarding processes

Maintain effective working relationships with partner organisations

· Develop, maintain, and coordinate effective working relationships with external agencies

· Promote the benefits of learning and progression across the partnership as a key element of Early Help

· Ensure that the project is delivered effectively and that programme outcomes are met

· Attend and contribute to external meetings including, but not limited to, partnership meetings, referral meetings and multi-agency meetings.

· Work with the team at Back on Track to maximise referral pathways into this and other services

Capture evidence and learning from the programme

· Maintain timely, good-quality case notes and safety plans

· Maintain accurate, timely records of activity and outcomes to meet any reporting requirements

· Contribute knowledge, evidence, and best practice to develop the Early Help for Adults Navigator service and to advance systems-change priorities.

· Share learning from the Early Help for Adults Programme to enhance Back on Track services (and vice versa)

· Prepare and draft case studies to enhance the qualitative evaluation of the programme.

Contribute to excellence and quality improvement

· Foster a culture of encouragement and peer support across the team, creating a work environment that keeps wellbeing at its heart

· Use learning from the programme to inform other local service provision and create systems change, paying particular attention to the voice of participants on the programme

· Contribute to Back on Track team meetings and the day-to-day running of in-centre activities

· Work in accordance with the Early Help for Adults service operating model
