26/09 Floating Support Worker (Merseyside)

As a Floating Support Worker, you will support teams across Sefton and Liverpool with rota cover for annual leave and sickness. 

The rota is flexible to cover sites and will be created 4 weeks in advance. For this role you must able to work across multiple services with a range of different diagnosis and be a team player. 

You will help to promote the health and wellbeing of those you support. 

This will include making sure people access the health services they need; have as healthy a diet as possible; and live in a home environment that supports their wellbeing. It is an interesting and rewarding role, where no two days are ever the same. 

This service needs people committed to delivering high-quality, person-centred support. Previous experience desirable but not essential. 

A good communicator, who can build valued relationships with others, you will be able to use your judgement and common sense to get the best possible results for each person you support. 

Your focus will always be on the person you are supporting, promoting their dignity, and ensuring the highest standards of confidentiality are maintained.

Hours: 37.5 hours per week                                                        

Salary: £23,870.55 per annum (additional sleep over payment)

Contract: Permanent 

Based in: Working flexibly across Imagines Merseyside Supported Accommodation and Outreach Services in Liverpool & Sefton

The main duties of the role are to

• Take part in helping people draw up their person-centred plan and follow the agreed action plan

• Work in partnership with others to allow independence of choice

• To act with integrity and to identify opportunities to celebrate achievements to allow a positive, respectful and trusting relationship to grow

• Support people to improve their confidence and self esteem

• Support people to make informed choices around a healthy lifestyle including physical health and improving self-care

• Support people to manage their own physical, mental health and wellbeing in a positive manner

• Support people to meet their leisure, work, social, emotional and spiritual needs

• Support people to access the community and take part in community activities

• Support people to manage finances to meet domestic responsibilities and personal aspirations

• Provide advice and support with domestic tasks and practical issues such as dealing with neighbours and keeping their house clean

• Support people to access other services

• Support people who may have alcohol and/or drug issues

• Support people to maintain the safety and security of their home

• To prompt individuals and undertake personal care duties as required

• To write up and record activities undertaken 

• To complete risk assessment in accordance with Imagine’s policy and procedure

• To report any significant change in an individual’s health, behaviour or wider situation to a manager and liaise with clinical teams. 

• To attend, as appropriate, case conferences, reviews etc.

Person Specification

• NVQ Level 2 in Health & Social Care/Care Certificate desirable or equivalent
• Experience of working in a community care setting is desirable

• Commitment to supporting people consistent with their rights, choices and responsibilities

• Demonstrate a positive attitude to people with mental health problems
• Ability to provide sympathetic, emotional and practical support to tenants
• Ability to support tenants to make informed choices within their daily lives
• An understanding of how to promote inclusion for people with mental health needs at all levels
• Domestic skills relating to general household tasks (cooking, cleaning, shopping and managing money)
• Knowledge of safety issues in a domestic setting
• Creative problem-solving skills
• Excellent verbal and written communication skills, paying attention to detail, whilst being able to deliver high quality, accurate work quickly.
• Understanding of person-centred working
• Working with and relating to people from a wide variety of backgrounds
• Working as a member of a team
• Reliable and trustworthy
