The Nelson Trust is looking for a dedicated RISE Practitioner to join our Women’s Community Services team in Sedgemoor.
As a RISE Practitioner, you’ll support women who have experienced, or are at risk of, having children removed into care.
You’ll deliver intensive, relationship-based support, working creatively and tenaciously to help women address complex needs and achieve lasting change.
Your work will be central to reducing the number of children entering the care system and improving outcomes for families.
Due to the nature of the work carried out in our Women’s Centres, this role is restricted to female applicants only in accordance with the provisions of the occupational requirement (Equality Act 2010, pursuant to Schedule 9, Part 1).
Location: Sedgemoor, Bridgwater Women’s Centre
Hours: 35 hours per week (flexibility required)
What You’ll Do
- Manage a caseload of women, delivering tailored interventions as part of an integrated health, therapeutic, and social support package.
- Use assertive outreach to engage women and support them in accessing services such as sexual health, housing, and mental health.
- Work from the women’s centre and satellite bases across Somerset as needed.
- Build and maintain effective partnerships with statutory and voluntary agencies.
- Ensure high standards of record-keeping, monitoring, and evaluation.
- Safeguard the welfare of children, young people, and vulnerable adults.
- Promote a culture of continuous learning and reflective practice within the team.
What We’re Looking For
Essential
- Experience working with people with complex needs, including those affected by substance misuse or involved in the criminal justice system.
- Ability to deliver high-quality services and work positively in partnership with a range of agencies.
- Car driver with access to a vehicle (travel across Somerset/Gloucestershire/Swindon/Bristol required).
- Strong administrative, IT, and record-keeping skills.
- Ability to manage challenging behaviour and respond flexibly to the demands of the role.
- Commitment to the values of respect, empowerment, and inclusion.
Desirable
- Knowledge of residential and community-based support practices.
- Experience working in the voluntary sector or supporting mentors.
- Understanding of the social care system and local community resources.
