Imagine Independence are looking for a canidate to provide person centred support to people with care needs living in Extra Care Housing which may include practical support, social support, personal care, support to manage finances and promote wellbeing.
Deliver care and support in accordance with support plans including personal care; administering and storing medication; shopping; meal preparation; domestic tasks; laundry; ironing and management of day to day finances.
Encourage and support clients to maximise self-care and independent living skills.
Support individuals to meet cultural and spiritual needs.
Support individuals to access community- based activities.
Promote health and wellbeing by supporting service users to attend health checks and appointments, take on board advice and guidance; promote healthy lifestyles.
Ensure Manager and practitioners are kept advised of concerns or significant changes in health conditions; including lack of engagement or non-compliance with treatment.
To enable individuals with memory loss to remain stimulated and orientated in time and place. To encourage positive, structured routines, which promote independence and maintain personal identity and social connections.
To support individuals in planning and experiencing dignified, comfortable and person-centred end of life care which meets their cultural and spiritual needs.
To support individuals to manage their accommodation and to maintain the safety, hygiene and comfort of their home.
To encourage individuals to effectively utilise any aids, adaptations or assistive technologies which promote their safety and independence.
