We are searching for a Support Worker in Nottingham to provide housing and property management related support to vulnerable individuals together with the aim of achieving a successful move-on and other measurable outcomes
Duties and responsibilities include but are not limited to:
·Supporting project workers with referral agencies, such as local authority, access point/gateway services, statutory and voluntary sector agencies in order to generate referrals for the service as required.
· Support with initial assessments for service users.
· Designing and delivering effective support plans, based on the service user’s needs and abilities.
· Maintain contact with service users as required depending on their needs maintaining a professional relationship and becoming a role model.
· Ensuring that where required housing and property management tasks are effectively undertaken in line with procedure and targets.
35 hours per week (+ 5 hour paid lunch) shifts 8:00 – 4:00 or 1:00 – 9:00 including every other weekend.
We are looking for someone to take their first step in the support of vulnerable people and help within their community, similar experience would be preferred.

A full driving licence and own vehicle would be desired
